I'm 5'3", 145lbs, 38DDD.  I've been to 6 weeks of PT (BCBS) requirement.  Didn't help, actually made my pain worse.  I'm scheduled for PS consultation Thursday Nov13th.  I'm excited & nervous!  I also know I will be very impatient waiting for a decision from ins co.  I've put this off for at least 5 years.  Scared as heck but also soooo ready.  I went shopping for clothes today & came back with nothing.  So sick of not fitting into anything!  It helps to know I'm one of so many others out there who just want normal size breasts.
